Making A Referral

Making a decision to seek treatment takes courage. Our experience indicates that once the decision has been made, the sooner the individual gets into treatment the better the outcome. If the process takes too long, many individuals start to second-guess their original commitment and this can have serious health consequences.

Avalon Centers is standing by every day to help you. You can call us at anytime and we’ll respond immediately to assist you.

First Step

Gather important information about the patient. We suggest the following:

  1. Patient History
  2. Basic Demographic Information
  3. Insurance Information

Second Step

Call Avalon Centers.

Avalon Eating Disorder Center
346 Harris Hill Road
Williamsville, NY 14221
Phone toll-free 1-866-814-0999, or local (716) 839-0999
Fax (716) 839-2058

Request to speak to an Intake Specialist – a psychiatric nurse will begin the process. The more information you can provide the nurse the quicker we can begin the admission process.

Third Step

If the patient appears to be appropriate for partial hospitalization, the patient contacts the Intake Specialist to set up an appointment for preadmission screening/assessment.
